Suprapubic catheter change

Change an established suprapubic catheter safely.

Safety first

Only staff who are trained and competent should change an SPC.
Confirm this is an established tract and that the first change has already been authorised/completed by Urology. If unsure, do not remove it—call the Surgical SHO/Urology team.
Have the correct replacement catheter, spare catheter, sterile water, drainage bag and emergency help available before removing the old catheter.

How to change it

Explain and consent. Check catheter type, size, balloon volume, allergies and when it was inserted.
Position supine. Clean and drape using aseptic non-touch technique.
Note the direction and insertion depth. Deflate the balloon completely and remove the catheter gently.
Insert the replacement immediately along the same tract and angle. Advance it about 3 cm further than the previous insertion depth.
Confirm urine drainage and correct position. Inflate the balloon with sterile water using the manufacturer’s stated volume, then withdraw gently until seated.
Reconnect, secure, check drainage and document catheter type, size, balloon volume, urine and any problems.

If replacement fails

The tract can close rapidly. Do not force the catheter or repeatedly probe the tract. Cover the site and call the Surgical SHO/Urology team immediately.
